Description and Services
Description (Service) Tele-cardiopulmonary rehab services for cardiac and pulmonary exercise class participants, in collaboration between the University of Ottawa Heart Institute, St Francis Memorial Hospital and the Lanark Renfrew Lung Program: 
* ongoing clinical cardiac rehab training 
* connects via Ontario Telehealth Network to OHI for pre-designated exercise sessions  
* education and resources for patients  
* cardiac rehabilitation nurses report, monitor, and evaluate participants’ progress 
* development of an individualized plan  
* self-management counselling 
* risk factor reduction 
* cardio-pulmonary conditioning 
* follow-up and collaboration with other care team members
