Description (Service) Management of acute and chronic foot conditions including assessment and treatment * nail surgeries for ingrown toenails * evaluation of current footwear * advice on proper footwear * Total Contact Casting (TCC) if appropriate

Eligibility People with diabetes and having open wounds and/or infections and who are without private insurance * individuals do not have to be a patient of the Family Health Team to receive service at this location
Application Referral from health care provider required * must use the Champlain Diabetes Chiropody Program referral form and criteria available on the website * no referrals accepted for routine care
