Description (Service) Program offers structured and supervised social * recreational and physical activities that are both meaningful and entertaining for the individual * provides a daily respite for those caring for an individual * transportation * nourishment and programming costs are included in the daily fee

Seniors and adults with disabilities * residents of Renfrew County * benefits most to people who are feeling lonely, bored, isolated or who live alone
Application Contact day program for information * Application to Adult Day Program is facilitated through the Champlain Local Health Integration Network (LHIN) 310-2222 * Upon receipt of an application, the Placement Coordination Services of the LHIN will determine eligibility
